Fetal responses to placental insufficiency: an update.
نویسنده
چکیده
Disturbance of normal fetal growth can result in a decrease in weight, or altered body proportion at birth. Fetuses that fail to reach their genetically predetermined growth potential due to intrauterine growth restriction (IUGR) following placental insufficiency are at increased risk for adverse short and long term outcomes that can extend all the way into adult life. Because of its diverse impacts, placental disease has been the research focus of a variety of biomedical subspecialties. Consequently, considerable information on IUGR has been published outside the obstetric core literature. Many of these observations may not appear salient in the context of clinical prenatal care. However, obstetricians and fetal medicine specialists are the primary caregivers to pinpoint, manage and co-ordinate research in these high risk pregnancies. Research and development on necessary new therapeutic strategies is most likely to be effective if the multiple interactions between fetal condition and outcomes can be identified and successfully modified prenatally. As these relationships are clarified, the focus of clinical fetal medicine is likely to shift from improvement of perinatal mortality and morbidity to enhancing the life-quality in survivors. It is the aim of this review to illustrate the multisystem fetal effects of placental insufficiency and how this information may stimulate future research.

Accurate assessment of gestational age, fetal growth, and the detection of fetal and placental abnormalities are major benefits of sonography. Color Doppler can be used to assist in the identification of vascular architecture, detection of vascular pathology and visualization of blood flow changes associated with physiologic processes and disease states. The clinical applications of obstetrical...
متن کاملThe placental pursuit for an adequate oxidant balance between the mother and the fetus
The placenta is the exchange organ that regulates metabolic processes between the mother and her developing fetus. The adequate function of this organ is clearly vital for a physiologic gestational process and a healthy baby as final outcome. The umbilico-placental vasculature has the capacity to respond to variations in the materno-fetal milieu. Depending on the intensity and the extensity of ...
متن کاملUpdate on the pathophysiological implications and clinical role of angiogenic factors in pregnancy.
Angiogenic markers are now being incorporated into clinical practice for the screening, diagnosing, and monitoring of preeclampsia. Pregnancy requires both vasculogenesis and angiogenesis in the fetal compartment and angiogenesis in the maternal compartment. Abnormal angiogenesis in the placenta determines impaired remodeling of the maternal spiral arteries and placental underperfusion that may...

During pregnancy parathyroid hormone-related protein (PTHrP) is one of many growth factors that play important roles to promote fetal growth and development including stimulating placental calcium transport. Angiotensin II, acting through the AT1a, receptor is also known to promote placental growth. We examined the effects of bilateral uterine artery and vein ligation (Restricted), which mimics...
متن کاملThe role of homeobox genes in the development of placental insufficiency.
Intrauterine growth restriction (IUGR) is an adverse pregnancy outcome associated with significant perinatal and pediatric morbidity and mortality, and an increased risk of chronic disease later in adult life. While a number of maternal, fetal and environmental factors are known causes of IUGR, the majority of IUGR cases are of unknown cause. These IUGR cases are frequently associated with plac...
